Medical History FormYour appointment time is set aside for you alone. Please notify us at least 24 hours in advance if you cannot attend your scheduled appointment so that we can offer the time to another patient.
Unless other arrangements have been made, payment is required at each appointment. We have HICAPS for direct billing of your health insurer. We also accept cash, credit card and EFTPOS.
